Surgery & Perioperative Clinical Calculators

Evidence-based surgery and perioperative calculators for pre-operative physical status (ASA PS), surgical wound classification, obstructive sleep apnea screening (STOP-BANG), venous thromboembolism risk (Caprini), and cardiac risk assessment (RCRI).

Why This Matters

Proper pre-operative risk assessment reduces perioperative complications and mortality. ASA PS is used in over 1 billion surgeries worldwide. Surgical site infections affect 2-5% of all surgeries. OSA is undiagnosed in 80% of surgical patients. VTE is a leading cause of preventable hospital death. Perioperative cardiac complications affect 1-5% of non-cardiac surgeries and are a leading cause of post-operative mortality.

When to Use

Use ASA PS for every surgical patient to document pre-operative physical status. Use Wound Classification for every operative procedure to stratify SSI risk. Use STOP-BANG for pre-operative OSA screening in all patients. Use Caprini Score for VTE risk stratification in surgical and hospitalized patients. Use RCRI for cardiac risk assessment before non-cardiac surgery.

Our surgery and perioperative calculators are developed in accordance with international guidelines including the ASA, CDC NNIS, ACCP/CHEST, AHA/ACC, and ESC/ESA guidelines.

Frequently Asked Questions

No. ASA class describes the patient's pre-operative physical status, not operability. Even ASA IV and V patients may benefit from life-saving or emergency surgery. However, higher ASA class increases perioperative risk and should prompt optimization and risk discussion.
Prophylactic antibiotics (Class I-II) are given to prevent infection before contamination occurs. Therapeutic antibiotics (Class III-IV) treat established infection or significant contamination. Class III may warrant extended prophylaxis or therapeutic course.
The Caprini score was developed for surgical patients. Medical patients should use alternative tools like the Padua Prediction Score or IMPROVE VTE risk assessment for VTE risk stratification.
RCRI was validated for elective non-cardiac surgery. For emergency surgery, NSQIP or institutional risk calculators may be more appropriate. However, RCRI risk factors still correlate with outcomes in emergency settings.
