Forced expiratory volume in 1 second (FEV1) and forced vital capacity (FVC) are essential pulmonary function measurements. The FEV1/FVC ratio distinguishes obstructive from restrictive patterns. A ratio <0.70 indicates obstruction (airflow limitation), while a ratio ≥0.70 suggests normal or restrictive physiology. GOLD criteria define COPD severity based on FEV1% predicted. FVC <80% with normal ratio suggests restriction.

Formula

FEV1/FVC Ratio = FEV1 (L) / FVC (L)

The ratio is calculated by dividing FEV1 by FVC, both measured in liters. A ratio ≥0.70 is considered normal. A ratio <0.70 indicates obstructive airway disease. If FEV1% predicted is provided, GOLD stage severity classification is applied.

A 68-year-old smoker: FEV1 1.2 L, FVC 2.8 L. Ratio = 1.2/2.8 = 0.43 (<0.70). FEV1% predicted = 45%. This indicates severe COPD (GOLD Stage 3). Recommend bronchodilator therapy, pulmonary rehabilitation, and smoking cessation.
