About

QUICKI was developed by Katz et al. in 2000 as an alternative to the hyperinsulinemic-euglycemic clamp, the gold standard for measuring insulin sensitivity. Unlike the clamp technique, which is labor-intensive, invasive, and impractical for clinical use, QUICKI provides a rapid assessment from a single fasting blood sample. It is calculated as the reciprocal of the sum of log-transformed fasting insulin and fasting glucose concentrations: QUICKI = 1 / (log(I₀) + log(G₀)), where I₀ is fasting insulin in μU/mL and G₀ is fasting glucose in mg/dL. QUICKI correlates well with clamp-derived insulin sensitivity (r = 0.7-0.9) and has been validated across diverse populations including normal, obese, diabetic, and polycystic ovary syndrome patients. Values >0.45 indicate normal insulin sensitivity, 0.35-0.45 indicate reduced insulin sensitivity, and <0.35 indicate severe insulin resistance. QUICKI is particularly useful for epidemiological studies and clinical research where clamp techniques are not feasible, and for monitoring changes in insulin sensitivity over time in response to interventions (weight loss, exercise, medications).

QUICKI is calculated from fasting insulin (μU/mL) and fasting glucose (mg/dL) using the formula above. Both values are log-transformed before summation. The result is a numeric value typically ranging from 0.25 to 0.55, with higher values indicating better insulin sensitivity. QUICKI can be used with either log₁₀ or natural log (ln) — the result is the same either way. The index has been validated against the gold-standard hyperinsulinemic-euglycemic clamp, demonstrating strong correlation particularly in non-diabetic individuals.

A 45-year-old woman with PCOS and BMI 32 kg/m² undergoes metabolic screening: fasting glucose 98 mg/dL, fasting insulin 18 μU/mL.
