For the IPI, extranodal involvement refers to discrete, non-contiguous extranodal disease sites. Direct extension from a nodal mass into adjacent extranodal tissue (e.g., a mediastinal mass extending into the lung) should not be counted as a separate extranodal site. True extranodal involvement includes separate organ involvement such as liver, lung parenchyma, bone marrow, pleura, pericardium, or gastrointestinal tract.
