Left ventricular ejection fraction (LVEF) is the most widely used measure of systolic cardiac function, derived from echocardiography, cardiac MRI, nuclear imaging, or left ventriculography. It is calculated as (stroke volume / end-diastolic volume) × 100%. The classification of LVEF into normal, borderline, and reduced categories is standardized by major society guidelines including the American Society of Echocardiography (ASE), the European Association of Cardiovascular Imaging (EACVI), the American College of Cardiology (ACC), and the European Society of Cardiology (ESC). The standard thresholds are: ≥55% normal, 50-54% borderline (or low-normal), 40-49% mildly reduced, 30-39% moderately reduced, and <30% severely reduced. The 2021 ESC guidelines for heart failure introduced the term "HF with mildly reduced EF (HFmrEF)" for LVEF 40-49%, recognizing the distinct pathophysiology and treatment response in this group. LVEF is crucial for diagnosis, prognosis, and treatment decisions in heart failure, guiding the use of ACE inhibitors, beta-blockers, mineralocorticoid receptor antagonists, SGLT2 inhibitors, ICD therapy, and CRT.

LVEF is the fraction of end-diastolic volume ejected during systole. A 2D echocardiogram using the biplane method of disks (Simpson's rule) is the standard method for LVEF measurement, where the endocardial border is traced in the apical 4-chamber and 2-chamber views at end-diastole and end-systole. Score Interpretation

LVEF is the single most important predictor of outcomes in patients with heart disease and is essential for guiding therapeutic decisions across cardiology. In heart failure, LVEF determines classification (HFrEF, HFmrEF, HFpEF) and guides pharmacotherapy. The landmark clinical trials establishing the mortality benefit of ACE inhibitors, beta-blockers, MRAs, and SGLT2 inhibitors were conducted in patients with reduced LVEF, and these therapies are guideline-directed for HFrEF (LVEF ≤40%). ICD therapy for primary prevention of sudden cardiac death is indicated when LVEF ≤35% despite optimal medical therapy for at least 3 months. CRT is indicated when LVEF ≤35% with LBBB and QRS ≥150 ms. Serial LVEF measurement is essential for monitoring disease progression or recovery.
